Course Description

The course is designed to train and assess participants in an incident investigations and analysis according to company's HSSE Incident Procedure. 

Course Content

·     Incident reporting and investigation procedure, tool and techniques such as fact- findings, interviews and verification, reporting SMART recommendations.

·     Accident mechanism – The Hazard Target Event triangle and cascading events

·     Accident Core Diagram – failed barriers and defenses

·     Tripod Accident Causation theory

·     Human factors in an accident

·     Incident investigation syndicate exercises and presentations
